The OHWP will have four levels that are determined by participation: Bronze (lowest level), Silver (low level), Gold (moderate level) and Platinum (highest level). All eligible employees, spouses or registered domestic partners and eligible retirees may participate in the OHWP Lunch & Learn Classes and Activity Classes. Only Eligible employees, spouses or registered domestic partners may earn the cash incentive; $25.00 for completing the Personal Wellness Profile and $25.00 for having a Biometric Screening. Only Eligible employees may earn the premium reduction. Eligibility is based on participation in one of the County Sponsored Health Plans - Exclusive Care, Kaiser or BlueShield.
 
Eligible employees who complete the Personal Wellness Profile, the Biometric Screening and 1 Action Plan will be granted a $30.00 per month premium reduction for the following benefit plan year. Eligible employees who complete the Personal Wellness Profile, the Biometric Screening and 2 Action Plans will be granted a $50.00 per month premium reduction for the following benefit plan year. While the program is completely voluntary, should the employee wish to earn the monthly premium reduction, the employee is required to first complete the Personal Wellness Profile and fully participate in the Biometric Screening and Action Plan(s).
